Visionwest Community Connection Service is a key provider in the west, supporting those self-isolationg with Covid or as close contacts in the community. They take reffereals through the Whānau Centre.

COMMUNITY CONNECTION SERVICE PROVIDERS
The Community Connection service takes an active approach to ensuring people needing help are able to access information, support and services across multiple government agencies and service providers.
The Community Connectors support the welfare needs of individuals and whānau to keep them safe while isolating at home / in the community. Community Connectors navigate and connect individuals and whānau to various services available during and in transition from self-isolation.
NORTHERN REGION HEALTH COORDINATION CENTRE
To help relieve pressure on emergency departments across a West Auckland medical hub has been established for people who have tested positive for COVID-19 and their isolating whānau, who require urgent, medical care that is not COVID-19 related.
Staffed by GPs and nurses with experience dealing in urgent care. This free service can be accessed by appointment only between 8am and 8pm, 7 days a week.
Referrals are accepted from emergency department clinical staff, the Whānau HQ co-ordination hub (which is NRHCC’s home isolation support centre), St John and Healthline.

He Poutama Rangatahi - Youth Employment Pathways is a cross-agency initiative aimed a rangatahi who are most at risk of long-term unemployment and who may need extra individualised and ongoing support to connect to training and employment.
